Informed Consent Agreements

Please be aware that by attending any session, Constellation or group event, you agree to these Terms.
If you are a new client and didn’t already agree to these terms online when booking then please fill out the Intake and Consent Form by clicking here, or request an email or paper copy from Medoc directly.
If the Consent Form changes significantly it may occasionally need to be completed by existing clients.
General Information
Medoc is a BodyTalk Practitioner, Constellation Facilitator, Hakomi practitioner, Reiki practitioner and LifeCoach. These are modalities that use energy, observation and awareness to support your overall health, wellbeing and spiritual growth. Medoc is not a licensed medical practitioner or licensed mental health therapist - she does not diagnose illness or disease, nor prescribe medications, supplements or treatments of any kind. Nor does Medoc make any promises about outcomes. Working with Medoc is not claimed to be a substitute for regular medical treatment or support from a mental health professional. Please seek additional professional help if the support you need goes beyond what she can provide.
Be aware that Constellations and sessions may include potentially sensitive content or highlight unresolved traumatic patterns that occur in individuals and families. Also, when done in person, Constellations or sessions may include close physical proximity or physical contact. By taking part you agree to take responsibility for your own wellbeing and physical comfort during sessions and group events, and to communicate to Medoc if you need something. You are welcome to end a session, or leave a group event at any time. One to one sessions or working online may be better for those who are more sensitive.
Medoc does not keep detailed notes of sessions, group events or Constellations. However any information you provide electronically (for example, when scheduling, emailing, paying or on consent forms) is recorded and may be saved. There is always a risk that information shared via phone, email or online may not remain confidential. Please help minimize this risk by providing minimal personal details and by not sharing sensitive/medical information.
Except in the case of gross negligence or malpractice, I or my representative(s) agree to fully release and hold harmless Medoc Perry, from and against any and all claims or liability of whatsoever kind or nature arising out of or in connection with in person or online: sessions, Constellations, group events and courses.
Payment, Rescheduling and Cancellation
Payment is required prior to any session or group event. Pay by credit card or paypal online. Or contact Medoc directly to pay by cash or check.
48 hours notice is required for cancellation or rescheduling and full payment will be required for shorter notice. More notice is appreciated so your session or spot can be offered to someone else. Reasonable exceptions will be made for illness or other unavoidable circumstances.
Payment made for group events, gift certificates or packages is non refundable but can be transferred to other events or services.
Packages and gift certificates must be used within 12 months.
Group Events
Creating a Safe Container
Creating a safe container is really important for doing any healing or ceremonial type work. Please only join a group event if you can take part in a good, supportive and attentive way.
If you are ill, intoxicated, trying to multitask, driving or have any condition where you are unable to give the group event your full attention while it is happening, then it is best not to attend.
To help events be inclusive for all people please interact with kindness and avoid discussion of potentially controversial or polarising topics unless they are relevant to the work being observed or your personal intention. Please keep your comments related to your own personal experience.
Constellations events are not recommended for children because potentially sensitive information may be shared. You can set a Constellation for your child without them being there.
Confidentiality
You are welcome to share your own personal learning from a group event - but please don't share any personal or sensitive information about other people, the healing work or the Constellations.
Please don't take any photos or make recordings.
The Following is for Online Group Events:
Please join the event on time from a quiet, private space with good internet connection. Be aware it may not be possible for you to join or re-enter the event once it has started if it would disrupt the work being done.
Recordings:
Medoc does record some online group sessions, and these recordings are shared with other registered participants and sometimes publicly. Usually participants don’t show on the recordings - But be aware if you attend the event live there is a possibility your video or name may show on a recording and be shared (usually in the case of a technical glitch). If you are concerned about privacy you can choose to keep your video off, and/or change the name shown on the screen, or wait for the recording instead of attending live.
Please do not record or take screen shots yourself, or share any recordings without Medoc's prior permission.
Constellation events are not recorded.
